ABOUT
All about
Thrive 360 Adult Day Center.
Thrive 360 Adult Day Center is a community-based program dedicated to supporting adults with disabilities through structured, person-centered services that promote independence, dignity, and whole-person wellness. Located in Sacramento, California, Thrive 360 provides a safe, inclusive, and engaging environment where individuals can build meaningful life skills, strengthen social connections, and work toward their personal goals as outlined in their Individual Program Plans (IPP/ISP).
Our program is designed to support the whole person—physically, emotionally, socially, intellectually, and functionally. Through daily participation in life skills training, health and wellness activities, community integration, and pre-vocational development, clients are empowered to increase independence and confidence in everyday life.
At the core of Thrive 360 is a commitment to person-centered planning. Every participant receives individualized support tailored to their strengths, needs, preferences, and long-term goals. We believe that every individual has the right to live with purpose, engage in their community, and experience meaningful growth at their own pace.
Thrive 360 collaborates closely with families, caregivers, and regional partners to ensure coordinated care and consistent progress. Our goal is to create a supportive environment where each client is valued, respected, and given the opportunity to thrive in all areas of life.
